DOCUMENTS REQUIRED FROM INTERN STUDENTS AT THE START OF THE INTERNSHIP
- Student Certificate (1 copy) (from e-Government or with a wet signature from the school)
- Photocopy of ID Card
- Photograph (1 piece)
- Social Security (SGK) Status Document
(4/A Employment Entry Declaration — must show that the current school internship insurance has been completed via e-Government. Students whose insurance continues from previous internships must have documents approved by the school with signature and stamp ing the continuation of their insurance.) - Medical Report
A single physician report stating: “Fit to work in hazardous and very hazardous jobs”
(Issued by Workplace Physician, General Practitioner, or Specialist) - Students whose names are approved by the Provincial Health Directorate will be directed to the workplace physician for this report.
- Students whose names are submitted to our hospital must first apply to the Training Unit for the medical report.
- The report can be obtained from our hospital’s workplace physician.
- Reports obtained from external institutions must be approved by the workplace physician to be valid.
- Interns must apply to the hospital Training Unit at least 1 week in advance for the medical report.
Required Tests(must be dated no more than 2 months before internship start):
- HBsAg, anti-HBs, anti-HCV, anti-HIV
- Hemogram, AST, ALT, Urea, Creatinine, Glucose
- Blood group results
- Chest X-ray
- Additional tests may be requested based on medical history and physical examination
- Vaccination card showing tetanus vaccination from Family Health Center
- Occupational Health and Safety (OHS) Training Certificate (if available) or tran
(Must be 16 hours and completed within the last 1 year) - Mandatory Internship Form / Internship Acceptance Form (2 copies) (for universities)
- Vocational Training Contract / Practical Internship Contract
- ID Badge (Student card)
- Uniform related to the internship department
- For Anesthesia, Perfusion, and Operating Room Services students:
- 1 set of green scrubs (top and bottom)
- 1 set of navy blue scrubs (to be worn outside the operating room)
- For EMT and paramedic interns: wearing a 112 emergency uniform is mandatory
Additional Department-Specific Requirements for Accepted Interns
- Nutrition and Dietetics students: Hygiene training certificate required
- Neonatal and Pediatric department students: Hepatitis A and chickenpox markers (Anti-HAV IgG, VZV IgG)
- Students in intensive care and immunosuppressed patient units (Hematology and organ transplant): Chickenpox marker (Anti VZV IgG)
- Students interning in Microbiology Laboratory: Hepatitis A marker (Anti HAV IgG)
